It is a general term that covers a large number of diseases, such as arthritis, rheumatoid arthritis, ankylosing spondylitis, osteoarthritis, gout, bursitis, trauma of joints, rheumatic fever which may have common symptoms like soreness, stiffness and pain in joints with swelling of tissues or muscles attached to these joints. Acute rheumatism has the symptoms of fever, stiffness, soreness, swelling and pain of joints and thus it is termed as rheumatic fever.
Rheumatic fever (acute rheumatism)
It is a disease that causes inflammation of connective tissue. Its origin is not known, but it is related to, and sometimes follows, certain specific bacterial infection of the throat or upper respiratory tract, like streptococci (see streptococcus) or associated with tonsillitis.
Rheumatic fever generally affects children of age group 5 to 15 years, or young adults, especially, who had rheumatic fever in their childhood, perhaps more than once. (Such patients also suffer from rheumatic heart disease.) In rheumatic fever there is a sudden onset of pain, swelling and stiffness of small and large joints (polyarthritis) with heat, redness of face, reddish circular patches on the trunk and shins. There is more sweating, tachycardia as well as malaise, fatigue and loss of weight. Sometimes there is chorea in little children. It has the tendency to recur - always worsein cold wet weather, which progresses into chronic rheumatism or chronic rheumatic heart. This illness develops within 2-3 weeks after an initial sore throat and cold in nose.
Rheumatic heart disease
Rheumatic fever has serious effects on the heart causing pericarditis and myocarditis. Chronic rheumatic heart disease may appear at age 2 to 50 years leading to heart failure, murmurs and damage to the valves of the heart.
Antibiotics are necessary before any future minor operations, such as tooth extraction, in order to prevent further damage of the heart.
Rheumatoid arthritis
It is a disease with unknown causes. There is chronic inflammation of various joints of the body, such as joints of the fingers, wrists, toes, ankles and often the knees, hips and shoulder, with severe pain and considerable swelling.
It is not just a disease of joints only but every system, tissue or part of the body is involved, suchas lungs, nerve tissues, and small arteries. Patient may have hypertensions, difficult breathing. Women are affected more than men. Eventually the affected joints may develop deformities. Non-articular rheumatism involves the soft tissues and causes fibrositis and lumbago.
